A note about physician referrals for home health services
Home health services — including skilled nursing, physical and occupational therapy, speech therapy, and home health aide care — are medically regulated and require a physician's order (referral) to be initiated. This requirement exists under Medicare, Medicaid, and most private insurance guidelines to ensure services are medically appropriate and properly covered.
If you believe home health services may be appropriate for you or your loved one, we encourage you to speak with your primary care physician, specialist, or hospital discharge planner and ask them to submit a referral to Zaytuna Home Health Agency. You can also complete this form so we have your information on file and can follow up once a referral is received.
